Subject Access Requests

You can request a copy of your medical records (Subject Access Report) for free; this has a 28-day turnaround from receipt of the request.

Non-NHS Services

The proper role of the GP is the care of patients who are, or believe themselves to be, ill; as a result, certain services are not included on the NHS, such as private certificates for employers, medical examinations for job applications, fitness to drive (HGV/PSV), etc.

When you hand requests to us, please check if a fee will be charged.

The fees charged are based on the British Medical Association (BMA) suggested scales and start at £15.00 and increase according to the amount of work involved and responsibility taken.

We do not offer this service on a Monday or a Tuesday (as these are our busiest clinical days).

Medical Records

When requesting access to medical records please be aware that we require this in writing. We have a duty of care to ensure that we are giving the correct records to the correct patient. We will also need at least 7 days notice.

Please Note: We now take card payments at all our surgeries except Skelton; payment is required prior to work being carried out.

Driving Medicals

HGV/PSV/Taxi/Sport/Pre-employment medical – Price on Application

As a part of medical requirements, we now require a copy of the driver’s latest eye test if they wear glasses or contact lenses. They will need to bring this to their Driver Medical appointment. If they fail to bring this to their appointment, they will not be able to receive their certificate.

The eye test must have been done within the last three months prior to their appointment. We also ask that the driver removes their contact lenses for their appointment, if they wear them.

Travel Vaccinations

If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements.

This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

Please be aware at times certain vaccinations may be unavailable due to national shortages.

There is further information about countries and vaccinations required on the links below:

Booking Your Appointment

It is important to make this initial appointment as early as possible – at least 6 weeks before you travel – as a second appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

We would like to notify you that our Monkgate Surgery is now a Yellow Fever Centre offering the vaccination to registered and non registered patients at a cost of £65.

If you are travelling to Europe a very useful booklet has been published with advice and guidance to help you get the most out of your holiday. To visit please click here.

This is a large document and may take a minute or two to view.
